Q: How do I know if my services require prior authorization?

A: Each insurance company varies in their requirements for pre-authorizations. Call your insurance company or refer to your insurance handbook for information on prior authorization. Note that your insurance company may not pay some services without prior authorization.

Q: Who is responsible for obtaining pre-authorizations?

A: It is ultimately your responsibility to ensure pre-authorizations are obtained before receiving services. The doctor and the hospital will make every effort to obtain or help obtain pre-authorizations, but you should always call your insurance company to make sure requirements are met.
