Eligibility Guidelines

Eligibility Guidelines

Tanner Hospice Care provides services for patients who meet the following criteria:

  • Have approval from and ongoing participation of their attending physician
  • Have a life expectancy of six months or less
  • Are seeking comfort-focused care rather than curative treatments
  • Have a responsible person available to assist with care at home

Hospice care is designed to support patients and their families with medical, emotional and spiritual care in familiar surroundings, promoting comfort and dignity at the end of life.

Service locations

Tanner Hospice Care is available to patients in the following west Georgia counties:

  • Carroll
  • Coweta
  • Douglas
  • Haralson
  • Heard
  • Paulding

Our team provides compassionate, around-the-clock care in the comfort of your home, helping patients and families through this important phase of life.
