Eligibility Guidelines

Patient eligibility for hospice

We offer hospice services for the following patients:

Patients who have approval from and continued participation of their attending doctor
Patients who have a life expectancy of six months or less
Patients who are seeking comfort-oriented care rather than curative treatments
Patients who have a responsible person available to assist in their care at home

Locations

Hospice care is available to patients in the following west Georgia counties:

Carroll
Coweta
Douglas
Haralson
Heard
Paulding
