Eligibility Guidelines

Patient eligibility for hospice

We offer hospice services for the following patients:

  • Patients who have approval from and continued participation of their attending doctor
  • Patients who have a life expectancy of six months or less
  • Patients who are seeking comfort-oriented care rather than curative treatments
  • Patients who have a responsible person available to assist in their care at home

Locations

Hospice care is available to patients in the following west Georgia counties:

  • Carroll
  • Coweta
  • Douglas
  • Haralson
  • Heard
  • Paulding
