Referral to palliative care

You may be referred to palliative care by any physician at Tanner, including a hospitalist, cancer specialist, heart specialist or neurologist. You also may be referred to the program by your primary care provider.

 Once referred, you’ll meet with specially trained clinical staff who will discuss your illness and listen to your needs and expectations. Our team will help you decide how best we can help you and your loved ones.
