Your Medicare Annual Wellness Visit is one of the most important yearly touchpoints for your health.

It’s designed to help you and your provider review your health, identify risks early and build a plan to support your long-term well-being.

Before the visit, Medicare requires all patients to complete a detailed questionnaire that covers their medical history, medications, daily routines, safety at home and more.

Many adults prefer to answer these personal questions independently and at their own pace.

Tanner MyChart makes it simple and convenient so you can avoid filling out long paper forms at check-in. You can complete the survey on your schedule.

If you’re unable to complete it before your appointment, you can pause it and finish later, or when you arrive for check-in.

The Medicare Annual Wellness Visit questionnaire is designed to help you and your providers stay up to date on your health. When you schedule your Annual Wellness Visit, Tanner MyChart allows you to:

Once you log in to Tanner MyChart, your upcoming appointments display on your home screen.

If your provider has assigned you the Medicare Wellness Visit Questionnaire, it appears during your eCheck-In process. If you don’t finish it the first time, the questionnaire stays available for up to seven days before your appointment.

These are the steps a Medicare patient will follow in Tanner MyChart when completing their questionnaire.

Open the appointment and tap eCheck-In.

Review each item one at a time. Some items require a simple verification. Others require a response about your health history, safety, lifestyle or daily activities.

If you want to take a break, select Finish Later. Your progress saves automatically so you can return anytime.

A status panel shows which questionnaires are completed and which are still in progress. You can reopen any questionnaire to finish it.

You may complete as much as you prefer in advance. If you still have items to complete on the day of your appointment, you can finish them during check-in.

When you arrive, the nurse will see your completed answers in your chart. This helps your care team begin the visit with the most accurate, up-to-date information.
