Tanner Health safely and responsibly collects, maintains and protects patient health information. We are committed to providing accurate medical records and patient information in a secure, efficient and timely manner while maintaining the confidentiality of every patient record.

Tanner Health offers several convenient options for requesting and receiving your medical records, including:

Accessing records through Tanner MyChart
Completing a self-service online medical records request
Printing and submitting an authorization form by email, fax or mail

Access Your Medical Records with Tanner MyChart

Tanner MyChart gives patients secure online access to portions of their medical and surgical history, including:

Lab results
Test results
Current immunizations
Provider notes

Patients also may request additional portions of their medical records through Tanner MyChart.

Tanner MyChart is free to use, secure and available 24 hours a day. Please allow up to 3 to 5 business days for records requests to be processed and released.

Request Your Medical Records Through Online Authorization

Medical records may be released to a patient or an authorized individual by completing an online medical records authorization form.

Please allow 7 to 10 business days for processing. Depending on the scope of the request, applicable fees may apply.

To submit an electronic medical records request, you must have:

A valid email address
A mobile phone
A driver’s license or government-issued photo ID for identity verification

Obtain copies of birth and death certificates

To obtain a copy of a birth or death certificate, you must contact the county's probate court office or health department in the county the birth or death occurred or from the Georgia or Alabama Department of Public Health.
