If you’re unable to complete it before your appointment, you can pause it and finish later, or when you arrive for check-in.

How to access the questionnaire
Once you log in to Tanner MyChart, your upcoming appointments display on your home screen.
If your provider has assigned you the Medicare Wellness Visit Questionnaire, it appears during your eCheck-In process. If you don’t finish it the first time, the questionnaire stays available for up to seven days before your appointment.

Open the appointment and tap eCheck-In.

Review each item one at a time. Some items require a simple verification. Others require a response about your health history, safety, lifestyle or daily activities.

If you want to take a break, select Finish Later. Your progress saves automatically so you can return anytime.

A status panel shows which questionnaires are completed and which are still in progress. You can reopen any questionnaire to finish it.

You may complete as much as you prefer in advance. If you still have items to complete on the day of your appointment, you can finish them during check-in.

When you arrive, the nurse will see your completed answers in your chart. This helps your care team begin the visit with the most accurate, up-to-date information.
