Tanner’s Complex Disease Management program brings together a multidisciplinary team to help patients and their loved ones understand and approach the many medical, emotional and social challenges that come with a serious illness.
No matter your age or the stage of your disease, Tanner’s Complex Disease Management program can help ease a patient’s symptoms and the side effects of the disease and provide his or her loved ones the extra support they need to make important and informed care decisions.
About the Program
Tanner’s Complex Disease Management program focuses on relieving the pain, symptoms and stress related to a disease. Participation in the program may begin when the patient is first diagnosed or at a later point in the disease progression, and care through the program takes place over an extended period of time.
The program — also called supportive care or symptom management — provides clinical, evidenced-based, palliative support for serious or complex disease, such as:
- Neurological diseases (such as ALS, Parkinson’s disease and others)
- Kidney or liver failure
- Lung disease or emphysema
- Congestive heart failure
- Chronic obstructive pulmonary disease (COPD)
The focus of the program isn’t only providing the latest clinical treatments, but supports curative treatment plans while helping to ease discomfort during ongoing illnesses.
Focusing on Quality of Life
The goal of Tanner’s Complex Disease Management program is to improve the quality of life for both the patient and his or her loved ones. The program can provide meaningful support regardless of the patient’s age or the stage of the disease.
The program brings together a multidisciplinary care team — including physicians, nurses, social workers, as well as pharmacists, nutritionists, chaplains and others — to help patients and their families make difficult decisions regarding care, disease progression and more. Complex Disease Management can help patients and their loved ones:
- Better understand a disease and the treatment options
- Weigh the risks and benefits of treatments or procedures
- Navigate difficult transitions (such as to a nursing home, comfort care or hospice program)
- Clarify and define surrogate decision makers or next of kin
- Advanced Care Planning, to work and hope for the best while having a plan for the worst
