Tanner Health System’s Get Healthy, Live Well offers a number of programs to help you on your journey to a healthier you. Designed for participants of any age, as well as their families and caregivers, these programs empower individuals with the knowledge they need to manage their disease and make informed choices in their daily lives.

Diabetes Education 

The following classes provide introductory-level education in diabetes, support to help you continue to keep your diabetes in check and additional resources for living well with diabetes. 

Diabetes Self-Management and Nutrition Education - Get Healthy, Live Well's diabetes self-management education program is certified by the American Diabetes Association. Each patient's needs are assessed by a registered nurse who is also a certified diabetes educator and a registered dietitian. A physician referral is required. 

Diabetes Prevention Program – The Diabetes Prevention Program, designed by the Centers of Disease Control and Prevention (CDC), is a series of classes that can help you make lasting changes to fight the key risk factors for developing diabetes. The program combines coaching from a certified lifestyle educator, proven learning materials and small-group support to set you on the path to better health.

Living Well With Diabetes – The Living Well With Diabetes program through Stanford University is designed for people living with diabetes. Get Healthy, Live Well offers a six-session Living Well With Diabetes workshop to help you manage diabetes.

Evidence-based Programs

Get Healthy, Live Well has made it a priority to build healthier communities with its evidence-based programs, proven to have a positive impact on those who participate. The programs include focused strategies that address the lack of access to chronic disease management and prevention. They also include strategies to address healthy eating, physical activity and quitting tobacco.

Freshstart – Want to quit tobacco? Get Healthy, Live Well offers Freshstart tobacco cessation classes through the American Cancer Society. This is your opportunity to kick the tobacco habit for good with the support you need. Free nicotine replacement may be available upon completion of the class.

Get Healthy Kids - Get Healthy Kids is a free, healthy lifestyle program for children ages 8 to 16 and their parents or guardians. The program is led by Get Healthy, Live Well staff and trained volunteers. It consists of six weekly, 90-minute sessions for the entire family. Each session includes a nutrition lesson, physical activity, goal setting and a support session for parents. 

Living Well Workshop – The Living Well Workshop through Stanford University is for those living with ongoing symptoms of chronic illness, such as fatigue, pain, tense muscles or shortness of breath. This six-session Living Well workshop will help you manage any chronic disease, including arthritis, cancer, depression, diabetes, heart disease, hypertension and more. Family members and informal caregivers are also encouraged to attend classes.

Tai Chi for Health - Get Healthy, Live Well offers Tai Chi for Health classes to help adults increase mobility, improve balance and physical function, and reduce stress.

Nutrition Services

Nutrition Services - With registered dietitians on staff, Get Healthy, Live Well is able to offer one-on-one nutrition education to patients needing help controlling their weight, blood pressure, cholesterol or diabetes.
