Tanner Health System’s Get Healthy, Live Well offers evidence-based programs in the community to help you on your journey to a healthier you. All programs are taught by certified lifestyle educators.
Diabetes Prevention Program – The Diabetes Prevention Program, designed by the Centers of Disease Control and Prevention (CDC), is a series of classes that can help you make lasting changes to fight the key risk factors for developing diabetes. The program combines coaching from a certified lifestyle educator, proven learning materials and small-group support to set you on the path to better health.
Freshstart – Want to quit tobacco? Get Healthy, Live Well offers Freshstart tobacco cessation classes through the American Cancer Society. This is your opportunity to kick the tobacco habit for good with the support you need. Free nicotine replacement may be available upon completion of the class.
Kids N Fitness – Kids N Fitness is a healthy lifestyle program for children ages 8 to 16 and their parents. The program —developed by a team of doctors and health professionals with Children's Hospital Los Angeles — consists of six, 90-minute sessions for the entire family and meets once a week. Each session includes a nutrition lesson, physical activity, goal-setting, support session for parents, a healthy snack and a healthy living incentive.
Living Well With Diabetes – The Living Well With Diabetes program through Stanford University is designed for people living with diabetes. Get Healthy, Live Well offers a six-session Living Well With Diabetes workshop to help you manage diabetes.
Living Well Workshop – The Living Well Workshop through Stanford University is for those living with ongoing symptoms of chronic illness, such as fatigue, pain, tense muscles or shortness of breath. This six-session Living Well workshop will help you manage any chronic disease, including arthritis, cancer, depression, diabetes, heart disease, hypertension and more. Family members and informal caregivers are also encouraged to attend classes.
Making Health a Priority with Evidence-Based Programs
Get Healthy, Live Well has made it a priority to build healthier communities with its evidence-based programs, proven to have a positive impact on those who participate. The programs include focused strategies that address the lack of access to chronic disease management and prevention. They also include strategies to address healthy eating, physical activity and quitting tobacco.
Through evidence-based programs, Get Healthy, Live Well is able to utilize group support to promote chronic disease prevention and self-management skills. These connections provide access to community resources and support to prevent, delay or manage their condition.
Get Healthy, Live Well is also working with churches and other community organizations to more effectively reach residents at disproportionate risk for chronic disease. With the help of its partners, Get Healthy, Live Well is bringing care and knowledge beyond the clinic and out into the community where people live, work and play.
