Making Health a Priority with Evidence-Based Programs

Get Healthy, Live Well has made it a priority to build healthier communities with its evidence-based programs, proven to have a positive impact on those who participate. The programs include focused strategies that address the lack of access to chronic disease management and prevention. They also include strategies to address healthy eating, physical activity and quitting tobacco.

Through evidence-based programs, Get Healthy, Live Well is able to utilize group support to promote chronic disease prevention and self-management skills. These connections provide access to community resources and support to prevent, delay or manage their condition.
Get Healthy, Live Well is also working with churches and other community organizations to more effectively reach residents at disproportionate risk for chronic disease.

With the help of its partners, Get Healthy, Live Well is bringing care and knowledge beyond the clinic and out into the community where people live, work and play.
