Tanner Health Source – More for Your Patients

Tanner Health Source is the community outreach program for Tanner Health System, providing Tanner with an innovative way to fulfill its mission of improving the general health of the community.

With exercise programs, nutrition education, screenings, community health fairs and more, Tanner Health Source provides physicians with innovative solutions for treating patients recently diagnosed with hypertension, diabetes and more.

Diabetes Education
Tanner Health Source offers a nationally-accredited diabetes education program. Designed for patients of any age, as well as their families and caregivers, Tanner Health Source empowers individuals with diabetes with the knowledge needed to manage their disease and make informed choices in their daily lives.

Tanner Health Source is staffed with a registered nurse and registered dietitian who specialize in diabetes management. Patients who have a confirmed diagnosis of diabetes will likely find that their health insurance plan or Medicare/Medicaid coverage reimburses most if not all diabetes management classes.

A physician referral is necessary for patients taking diabetes education classes at Tanner Health Source, and all services are offered by appointment only. To make a referral, call 770.838.8367.

Exercise Classes and Programs
Tanner Health Source offers certified health education specialists, including an exercise specialist to help patients develop an exercise regimen and adopt a more active lifestyle.

Tanner Health Source also administers the Tanner Walking Program, a free community-based program that encourages participants to track how much they walk and provides incentives – such as pedometers and water bottles – when certain milestones are met. Participants are encouraged to walk anywhere they are comfortable, and Tanner provides two community daylight walking trails located at Tanner Medical Center/Carrollton and Tanner Medical Center/Villa Rica.

Health Education
Tanner Health Source programs focus on changing participants’ lifestyle, encouraging them to adopt healthy habits.

The program takes the lead on improving the health of the community by organizing public health fairs and hosting classes and seminars to educate attendees on a wide range of health information. Nutritionist Services
With registered dietitians on staff, Tanner Health Source is able to offer one-on-one nutrition education to patients needing help controlling their weight, blood pressure, cholesterol or diabetes.

Dietitians also conduct regular nutrition education classes to help educate larger groups on the merits of a healthy diet and how to obtain better nutrition.
